After you have scheduled your first appointment, you can:

• Open  the intake forms to print, fill out, and fax back to CAPC, LLC at 414.485-4412
• You may choose to fax a copy of your insurance cards as well.
• Review the Notice of Privacy Practices
• The Guidelines for Patients at CAPC is a helpful tool for knowing how to manage appointments, refills, and emergency communication
