After you have scheduled your first appointment, you can:

1. Open  the intake forms to print, fill out, and fax back to CAPC, LLC at 414.485-4412
2. You may choose to fax a copy of your insurance cards as well.
3. Review the Notice of Privacy Practices
4. The Guidelines for Patients at CAPC is a helpful tool for knowing how to manage appointments, refills, and emergency communication
5. If you wish to communicate by email, please print, sign and return the consent to communicate by electronic mail
