Instructions for Using the Online Scheduler

1. The online scheduler works best in the mozilla firefox internet browser. It tends to fail in other browsers!
2. For additional information about scheduling new intakes or appointments, click HERE
3. When you use the online scheduler, be sure to fill in every heading. If you don’t have a mobile phone, add your home phone twice, and if your mobile phone is  your home phone, then enter it twice
4. Be sure to confirm your  appointment by clicking on the link sent to  you by email.  If you do not receive this email, something went wrong. If you do not click the confirmation link you have NOT scheduled your appointment.

If you are a new patient:

• You will need to call us to schedule your intake appointment: 715-318-0047
• It will be very helpful if you do the following:
  1. Verify with your insurance that this office is in network (Child and Adolescent Psychiatry Consulting, LLC Marshfield, WI 54449)
  2. Complete the intake forms
  3. Review the privacy practices and guidelines for patients
  4. Fax your completed copy of the intake forms and a copy of your insurance card to  CAPC at 920-694-3271
  5. If you wish to communicate by email, please sign and return the Consent to Communicate by Electronic Mail.

Dr Jenna works in a number of locations, and contracts with different insurances at each location. Please note that it is not enough simply to see her name in your insurance network. You have to make sure she is in your network at the location you plan to go to for appointments.
